Fijne kerstdagen forumleden! Bedankt Galapagos voor de bijdrage aan ons kerstdiner. Veel belangrijker wordt in 2020 het spoedig vermarkten van Filgotinib in de laatste plaats voor ons aandeelhouders. Op de eerste plaats voor de mensen met reumaklachten. Die wachten met smart op een orale medicatie met weinig bijwerkingen.

Als filgotinib geen BBW krijgt zal Gilead dit wel uitmelken in de marketing, Eisai hoeft daar niets aan te doen. De distributie revenues zijn vaak ook te gering om grote marketing inspanningen te verrichten.

Gilead gets ball rolling on JAKs game in Japan

Gilead is gearing up for a full JAK1 launch.

Four days after submitting an NDA for their new JAK1 inhibitor for rheumatoid arthritis, filgotinib, the pharma giant has handed distribution and co-promotion rights in Japan to Eisai, should it be approved on the islands. Gilead has already submitted the drug for regulatory approval there.

Dollar terms of the deal were not announced, though they are likely sizeable given the drug’s blockbuster projections. Global peak sales estimates have ranged as high as $6 billion and tend to come in at around $3 billion. EvaluatePharma has offered a more conservative $1.1 billion.

Still, the drug may not be as valuable as it once was. Gilead and co-developer Galapagos had hoped to leverage the JAK inhibitor against a host of diseases but recent trial failures have dampened those hopes. In October, two Phase II trials failed lupus and Sjögren’s.

The Eisai rights cover marketing for any other indications filgotinib is ultimately approved for. Gilead is currently in Phase III with the drug for Crohn’s disease, ulcerative colitis, and psoriatic arthritis. Rheumatoid arthritis has long been seen as the most lucrative indication.

Filgotinib has also been submitted for EMA approval, where Gilead and Galapagos have divided marketing rights and responsibilities by country.

Late to the JAK game, it remains to be seen how filgotinib will compete against other such inhibitors on the market, including AbbVie’s newly approved upadacitinib. Proponents of the drug argue that despite slightly worse efficacy data, filgotinib will stand out for better safety data. Others have suggested the FDA could slap on the black-box warning the agency gave to AbbVie, treating JAK inhibitors as a class.

For Eisai, the news comes two days after the FDA approved their Dayvigo insomnia drug.

Gilead and Eisai ink pact on co-promotion of filgotinib

US biotech major Gilead Sciences’ (Nasdaq: GILD) Japanese subsidiary and Eisai (TYO: 4523) have entered into an agreement for the distribution and co-promotion of filgotinib, an investigational, oral, selective JAK1 inhibitor, in Japan, pending regulatory approval for the treatment of rheumatoid arthritis (RA).

Through this collaboration, Gilead Japan will retain responsibility for manufacturing and marketing approval of filgotinib, while Eisai will be responsible for product distribution in Japan in RA and other potential future indications. The companies will jointly commercialize the medicine if approved.

Approximately 600,000 to 1 million people are living with RA across Japan, and despite available options, many still do not experience disease remission. In the global Phase III FINCH studies, filgotinib demonstrated durable efficacy and safety results across multiple RA patient populations, including in people with prior inadequate response to methotrexate treatment (MTX), those who were intolerant to one or more biologic treatments and those who were MTX treatment-naïve.

"We are very pleased to announce this important new partnership with Eisai, which brings together our complementary expertise and commitment in inflammation, to deliver this important new option to patients living with inflammatory diseases in Japan," said Dr Luc Hermans, president and representative director, Gilead Japan.

"We have extensive clinical development and commercialization experience spanning more than 20 years in RA and have established a solid RA franchise in Japan," said Hidenori Yabune, president of Eisai Japan, senior vice president of Eisai, adding: "With this agreement, we look forward to contributing more to patients living with RA by adding filgotinib to our product line-up."

Gilead and Belgium-based Galapagos (Euronext: GLPG) entered into a global collaboration for the development and commercialization of filgotinib in inflammatory indications in 2015, which was expanded earlier this year. Filgotinib is an investigational drug whose efficacy and safety have not been established.

Filgotinib is pending regulatory approval in Japan, where a marketing application was filed in October, Europe and the USA, based on global Phase III trials evaluating its efficacy and tolerability.

Blockbuster sales potential forecast
Assuming eventual regulatory approval, filgotinib would likely be the fourth JAK inhibitor for RA after Pfizer’s (NYSE: PFE) $1.8 billion seller Xeljanz (tofacitinib), Eli Lilly’s (NYSE: LLY) Olumiant (baricitinib), and AbbVie’s upadacitinib which has already been submitted for approval with a verdict expected in the third quarter.

However, Jefferies analysts have suggested that filgotinib seems to have a superior safety profile, which could help it achieve peak sales of $6 billion at peak, with around half of that coming from RA and the rest from other diseases such as psoriatic arthritis, Crohn's disease, ulcerative colitis, ankylosing spondylitis and atopic dermatitis.
